Required Clearances

**** All results will need to be provided to Hosanna Industries at least three weeks prior to your arrival to volunteer.

​

A.  If volunteer is from Pennsylvania and has lived in the state for at least ten years.

1. Report of criminal history from the Pennsylvania State Police

2. Child Abuse History Clearance

3. Volunteer FBI Clearance Disclosure Statement.

B.  If volunteer is from Pennsylvania and has lived in the state for less than ten years.

1. Report of criminal history from the Pennsylvania State Police

2. Child Abuse History Clearance

3. FBI clearances. Follow these steps for the FBI Clearances:

• Visit IdentoGo

• Use code:  1KG6ZJ

• Schedule or Manage Appointment

• Fill out all the forms. *Note: Legal name must match exactly on all identification documents

• Select a site and schedule an appointment to be fingerprinted

• Take your ID and payment (they accept debit/credit or money order)

C.  If a volunteer is from outside of Pennsylvania

1. clearances from their own state, if any are required

2. FBI clearances.  Follow these steps for the FBI Clearances:

• Visit IdentoGo

• Use code: 1KG6ZJ

• Submit a Fingerprint Card by Mail

• Read the warning and hit continue

• Fill out the  forms. *Note: Legal name must match exactly on all identification documents

• Pay online and print the form

• Take this form to a local facility where you can get fingerprinted – the police department is an alternative

• Mail one set of fingerprint cards with the form, previously printed, addressed to you. *Note: Address will not be provided unless payment is completed
